I agree to declare any pre-existing condition which may require a declaration to a health authority or require accommodations for me to be able to train.

In addition to the above, if I am a candidate for admission to the Schulich School of Music, I consent to the use of my name and images in public recognition of academic achievement and in the advertising and audio and video recording of student ensemble concerts for distribution using different media and formats.

ACKNOWLEDGEMENT
